Required information. During the registration process, Portland Little League is required by Little League International to collect the following information on a player in order to complete the registration process:
  • Player's DOB
  • Player's School
  • Player's T-shirt size
  • Two Emergency Contacts for the Player (Names, phone numbers and relationship to player)
  • Parent's Medical Insurance Company, Policy Number, and Group Number
  • Medical or Health related diagnoses, conditions, allergies, tetanus and any medications the league should know for the safety of your child
  • Family physician name, address, and phone number
  • Hospital preference; in case of emergency
  • Photo Release and Medical Release statements

You must complete all sections noted with an asterisk (*)
Sections requiring uploading of documents may be skipped at the time of registration, but you must return and upload before your child is allowed to participate in practices or games.
Unfortunately, you will need to complete information and upload documents for each child being registered. This data is tied to individual players not parent or account.

Uploading Documents During Registration. The system has a 1MB file size limitation for uploading documents. We recommend scanning documents to PDF with scanner settings that ensure the PDF is less than 1MB. If you decide to photograph documents make sure each file is under 1MB. A good way to do this is to use the free Genius Scan app for iPhone or Android. Be sure to use "Single" mode. After photographing, tap the share icon and change the File size setting to be under 1MB. Then save the document to your device or e-mail it to yourself to get it to the device where you plan to perform your registrations.

Proof of Age. You must provide a copy (not an original) of the player's birth certificate issued by the state, county, or municipal government. There is a step in the registration process where you can upload a PDF or image of the birth certificate. It must be a clearly legible document. This is required for new players. Players interested in being considered for Tournament Play (All-stars) should also provide Proof of Age, if not provided in 2018.

Proof of Residency or School Enrollment. Little League International requires EITHER proof that the player resides in the city of Portland OR proof that the player is enrolled at a school in the city of Portland as of 9/30/18. We recommend The School Enrollment form, as it is the easiest!
  • If you choose to use proof of enrollment at a school in Portland, provide a copy of a completed School Enrollment Form:
  • The School Enrollment Form must be signed by the parent/guardian AND a School Administrator (Principal or Vice Principal). If you provide the signed School Enrollment form you do NOT need to submit any proof of residence/address documents.
  • Click here for the  School Enrollment Form
  • Complete the form, make sure it is signed by parent and Administrator, save it and upload. YOU CAN COMPLETE REGISTRATION WITHOUT UPLOADING THIS FORM, BUT IT MUST BE UPLOADED OR MAILED TO PLL PRIOR TO THE START OF THE SEASON

If you choose to use proof of residency in Portland, instead of School Enrollment, you will need to provide copies of 3 items showing a physical address in Portland (documents with P.O. Boxes are not valid for this purpose), dated or in force between February 1, 2018 and February 1, 2019. Each of the 3 items must come from a DIFFERENT Group (see below); for example, an electric and cable bill will only count as one of the 3 items. This is a Little League International Stipulation Not Portland Little League.
